The legal prescription path for ED medicines
In the United States, sildenafil, tadalafil, vardenafil, and avanafil are prescription drugs. The prescription is not a formality. It is the step that looks for nitrates, unstable heart disease, and interacting medicines before a vasodilator is dispensed. A seller who skips that step is not offering convenience. They are skipping the safety check this whole site is about.
What a competent evaluation covers
- The story of the symptom. Sudden or gradual, present on waking or only with a partner, present with every partner or only one, desire present or absent, pain, curvature, or a deformity. Those details separate vascular disease, performance anxiety, low desire, and Peyronie’s disease, which are treated differently. See psychological and physical causes.
- Heart and exercise tolerance. Chest pain, breathlessness, fainting, prior heart attack, stroke, heart failure, and blood pressure. If ordinary effort already causes symptoms, sexual activity may be unsafe until that is evaluated. The exclusions used in drug trials are on the cardiac page.
- A complete medicine and substance list. Nitrates in every form, poppers, riociguat, vericiguat, alpha-blockers, HIV medicines, azole antifungals, macrolide antibiotics, other blood-pressure drugs, antidepressants, and supplements. People forget sprays and “weekend” pills. The clinician cannot.
- Other contributors. Diabetes, smoking, alcohol, sleep apnea, pelvic surgery, prostate treatment, and depression. ED can be the first outward sign of vascular disease. Treating only the erection and ignoring glucose or blood pressure misses the larger risk.
- Selective tests, not a ritual panel. Blood pressure belongs in the visit. Glucose or A1c, lipids, and a morning testosterone are added when the history suggests them, not because every ED conversation requires every lab. Testosterone treatment is for testosterone deficiency, not a default add-on to a PDE5 inhibitor.
Telehealth that is real, and telehealth that is a storefront
A lawful telehealth visit is done by a clinician licensed where you are, who can refuse to prescribe, who documents a history, and who sends a prescription to a pharmacy you can verify. It can be appropriate for a straightforward case after screening questions that would catch nitrate use and unstable symptoms.
It is not a visit if the site cannot say no, never asks about chest pain or nitrates, ships the drug itself from an unnamed source, or offers a prescription as a line item in the cart. Those are the rogue patterns on the pharmacy page.
After the prescription
Fill it at a licensed pharmacy. Ask the cash price of the FDA-approved generic before you pay for a brand, unless the prescriber had a reason to write the brand. Read the leaflet’s nitrate warning even if you have taken the drug before. Go back if the first tries fail for a reason you can describe (heavy meals, no stimulation, fear of failure), rather than increasing the dose yourself. Go back sooner for chest pain, fainting, vision or hearing changes, or a prolonged erection.
Partners are part of the history when you want them to be. A prescription does not obligate you to explain your medical care to anyone else. Discretion is compatible with doing this legally. What discretion cannot include is a nitrate you have not mentioned.
